BY REGISTERING ONLINE YOU ACCEPT THE FOLLOWING: Participation in any Dreams In Motion activity and use of any recreational facilities involves a risk of accidental injury despite all safety precautions. Having been informed of the activities conducted by Dreams In Motion, I/we, as an individual or as a parent or guardian of the participants named herein, assume all risks and hazards incidental to the activities, and release from responsibility and agreed to indemnify and hold harmless Dreams In Motion, its officers, directors, independent contractors, volunteers and all employees for any illness or injury to me or my children or family members occurring during his/her/our participation in any activity or use of any recreational facility at or conducted by Dreams In Motion. I hereby give permission for any and all medical attention to be administered to me or my child by Dreams In Motion faculty or staff in the event of an accident, injury, sickness, etc until such time as I may be contacted. I also assume the responsibility for the payment of such treatment.
I also agree to abide by all policies of Dreams In Motion as set out here.
Dreams In Motion uses students photos for publicity on our website, in newspaper articles and in our print materials; if you do not wish for your child’s likeness to be used in any of these please make a note in the notes section on registration form.
